Recent wetenschappelijk onderzoek naar sauna therapie, warmte-therapie

Zeggen dat er geen wetenschappelijk onderzoek bestaat naar sauna-therapie is ronduit leugenachtig. We gingen op zoek in engelstalige medische databanken en vonden een schat aan zeer recent wetenschappelijk onderzoek (voornamelijk 2007-2008). We hebben de resultaten geordend volgens onderwerp. De drie databanken die gelijkaardige resultaten opleveren zijn:

  2. Journal of Psychosomatic Research
  3. PubMed, A service of the U.S. National Library of Medicine and the National Institutes of Health

De zoektermen die gebruikt werden zijn de volgende:

  1. Sauna
  2. LTSB: Low Temperature Sauna Bathing
  3. FIR: Far Infrared Radiation
  4. WAON Therapy
  5. Repeated sauna therapy

Let wel, het gaat hier niet over zo nu en dan eens een sauna-bezoek om de week of om de maand. Het gaat hier veeler over sauna-therapie, met dagelijkse sauna-sessies gedurende een zekere periode, ter voorkoming, verlichting of genezing van aandoeningen. Onze stelling is dat als er een therapie bestaat op basis van sauna of infrarood-sauna, de heilzame werking ervan in elk geval bewezen is.

We geven eerst een korte samenvatting per categorie van onderzoeken. Vervolgens citeren we voor medisch geschoolde lezers de oorspronkelijke Engelstalige conclusie van elk onderzoek. Er wordt steeds een link voorzien naar het wetenschappelijk artikel, waar soms full-text versies voorhanden zijn.

Hart en bloedvaten

Verschillende onderzoeken wijzen uit dat er positieve resultaten bestaan voor aandoeningen van hart en bloedvaten, o.a. tegen ontsteking van endotheel in bloedvaten, hartfalen, atherosclerose (aderverkalking),

  1. Far infrared therapy inhibits vascular endothelial inflammation via the induction of heme oxygenase-1.[Arterioscler Thromb Vasc Biol. 2008] - PubMed
    These results demonstrate that FIR therapy exerts a potent antiinflammatory effect via the induction of HO-1. The ability of FIR therapy to inhibit inflammation may play a critical role in preserving blood flow and patency of AVFs in hemodialysis patients.
  2. Far-infrared therapy: a novel treatment to improve access blood flow and unassisted patency of arteriovenous fistula in hemodialysis patients [J Am Soc Nephrol. 2007] - PubMed
    FIR therapy, a noninvasive and convenient therapeutic modality, can improve Qa and survival of the AVF in HD patients through both its thermal and its nonthermal effects.
  3. Thermal therapy: a viable adjunct in the treatment of heart failure? [Congest Heart Fail. 2008 Jul-Aug] - PubMed
    Significant improvements were noted across a wide scope of heart failure-related parameters in the areas of (1) endothelial function, (2) hemodynamics, (3) cardiac geometry, (4) neurohormonal markers, and (5) quality of life. Of special note, thermal therapy also conveyed a strong antiarrhythmic effect in heart failure patients. The clinical evidence highlights repeatable and compelling data showing that thermal therapy may provide an important and viable adjunct in the treatment of heart failure.

  5. Repeated sauna therapy increases arterial endothelial nitric oxide synthase expression and nitric oxide production in cardiomyopathic hamsters [Circ J. 2005] - PubMed

  6. Repeated sauna therapy reduces urinary 8-epi-prostaglandin F(2alpha).[Jpn Heart J. 2004] - PubMed
    These results suggest that repeated sauna therapy may protect against oxidative stress, which leads to the prevention of atherosclerosis.
  7. Repeated thermal therapy improves impaired vascular endothelial function in patients with coronary risk factors [J Am Coll Cardiol. 2001] - PubMed
    Repeated sauna treatment improves impaired vascular endothelial function in the setting of coronary risk factors, suggesting a therapeutic role for sauna treatment in patients with risk factors for atherosclerosis.

Artrose (Eng.:osteoarthritis) - reuma - Spondylitis Ankylosans ( Ziekte van Bechterew)

Twee onderzoeken (één met dieren en één met mensen) tonen aan dat infrarood nuttig is in de behandeling van artrose door het vertragen van de afbraak van kraakbeen, en door het verlichten van pijn en stijfheid.

  1. Infrared radiation and magnetic field therapy ameliorates cartilage damage in rabbits with knee osteoarthritis [Nan Fang Yi Ke Da Xue Xue Bao. 2007] - PubMed
    Combined infrared and magnetic field therapy can effectively alleviate cartilage destruction, shortens the disease course and enhance the therapeutic effects in rabbits with knee osteoarthritis
  2. Infrared sauna in patients with rheumatoid arthritis and ankylosing spondylitis. A pilot study showing good tolerance, short-term improvement of pain and stiffness, and a trend towards long-term beneficial effects. [Clin Rheumatol. 2009] - PubMed
    In conclusion, infrared treatment has statistically significant short-term beneficial effects and clinically relevant period effects during treatment in RA and AS patients without enhancing disease activity. IR has good tolerability and no adverse effects.

Hooikoorts (Eng.: allergic rhinitis)

Dit is wel een erg overtuigend onderzoek waarbij zowat alle symptomen van hooikoorts werden verlicht: tranende ogen, loopneus, verstopte neus en niezen.

  1. Clinical effects of far-infrared therapy in patients with allergic rhinitis [Conf Proc IEEE Eng Med Biol Soc. 2007] - PubMed
    During the period of FIR therapy, the symptoms of eye itching, nasal itching, nasal stuffiness, rhinorrhea and sneezing were all significantly improved. Smell impairment was not improved until after the last treatment. No obvious adverse effect was observed in the patients during treatment and follow-up. We concluded that FIR therapy could improve the symptoms of AR and might serve as a novel treatment modality for AR.

Lage rugpijnen

Deze studie geeft overtuigende resultaten in de behandeling van chronische lage rugpijnen.

  1. Infrared therapy for chronic low back pain: a randomized, controlled trial [Pain Res Manag. 2006] - PubMed
    The IR therapy unit used was demonstrated to be effective in reducing chronic low back pain, and no adverse effects were observed.

Huidaandoeningen

Over het effect op de huid werden er positieve resultaten vastgesteld op het vlak van:

  • een betere doorbloeding van de huid,
  • betere bescherming tegen zonlicht (UV-straling),
  • een betere fysiologie (vocht- en zuurgehalte) van de hoornlaag (stratum corneum), de buitenste laag van de huid die het organisme beschermt tegen uitdroging en die een effectieve barrière is tegen veel stoffen,
  • behandeling van sporotrichose, een infectieziekte van de huid.

    In conclusion, FIR therapy exerts a NO-related biological effect to increase skin microcirculation in rats. This might bring into perspective the clinical application of FIR to treat ischemic disease by augmenting L-arginine/NO pathway

  2. Infrared Therapy Could Offer UVB Protection: Use of light-emitting diode device may protect skin from excess UVB as well as sunscreen with SPF 15.
    All of the treatment regimens studies showed some ultraviolet B protection, with the most consistent protection coming against the effects of exposure to MED1 and MED2. The level of protection seemed to be dose dependent, with the best protection provided by a regimen of 10 infrared treatments in 1 week
  3. Effect of regular sauna on epidermal barrier function and stratum corneum water-holding capacity in vivo in humans: a controlled study [Dermatology. 2008] - PubMed
    The present data suggest a protective effect of regular sauna on skin physiology, especially surface pH and stratum corneum water-holding capacity. (c) 2008 S. Karger AG, Basel.
  4. Hyperthermic treatment of sporotrichosis: experimental use of infrared and far infrared rays [1993]
    Infrared and far infrared ray treatments allow the length of a single treatment to be reduced by three-quarters, in comparison with one pocket warmer treatment.

Decompressie-ziekte bij duikers

Een sauna-sessie voor een duik vermindert aanzienlijk het risico van decompressie-ziekte bij duikers.

    A single predive sauna session significantly decreases circulating bubbles after a chamber dive. This may reduce the risk of decompression sickness. Sweat dehydration, HSP, and the NO pathway could be involved in this protective effect.

Chronisch vermoeidheidssyndroom

Deze studies "laten vermoeden" dat herhaalde warmte-therapie o.a. via infrarood warmte, een veelbelovende methode kan worden in de behandeling van het chronisch vermoeidheidssyndroom (CVS).

  1. The effects of repeated thermal therapy for two patients with chronic fatigue syndrome
    These results suggest that repeated thermal therapy might be a promising method for the treatment of CFS (chronic fatigue syndrome).
  2. A new treatment: thermal therapy for chronic fatigue syndrome [Nippon Rinsho. 2007] - PubMed
    Thermal therapy using far-infrared ray dry sauna was performed for patients with chronic fatigue syndrome (CFS). Symptoms such as fatigue, pain, and low-grade fever were dramatically improved on two patients. And prednisolone administration was discontinued and became socially rehabilitated 6 months after discharge. On other 11 patients with CFS, physical symptoms such as fatigue and pain improved, too. Furthermore, we reported that repeated thermal therapy had relaxation effect and diminishes appetite loss and subjective complaints in mildly depressed patients. These results suggest that repeated thermal therapy may be a promising method for the treatment of CFS.

Beweging - motoriek

Verbetering van de perifere bloedsomloop bij patiënten met ernstige motorische of verstandelijke handicap.

  1. Effects of single low-temperature sauna bathing in patients with severe motor and intellectual disabilities. [Int J Biometeorol. 2008] - PubMed
    There was significant improvement in the parameters that are indicative of the peripheral circulatory status, including the skin blood flow, blood flow velocity, pulsatile index, and resistive index. Numbness and chronic myalgia of the extremities decreased. There were no adverse side effects. Thus, it can be concluded that LTSB improves the peripheral circulation in CP patients.LTSB= Low Temperature Sauna Bathing

Lichte depressie - verminderde eetlust

Het eerste onderzoek besluit dat herhaalde warmte-therapie nuttig is bij licht depressieve patiënten met subjectieve klachten en verlies aan eetlust.

Het tweede onderzoek is een al iets oudere studie die suggereert dat herhaalde sauna-therapie goed is voor allerhande ziekten die verband houden met de levenswijze van de persoon, zoals cardio-vasculaire aandoeningen, overgewicht (vermageren dus), rookverslaving, hoge bloeddruk.

    These findings suggest that repeated thermal therapy may be useful for mildly depressed patients with appetite loss and subjective complaints.
  2. Clinical implications of thermal therapy in lifestyle-related diseases[Exp Biol Med (Maywood). 2003] - PubMed
    It was found that repeated sauna therapy (60 degrees C for 15 min) improved hemodynamic parameters, clinical symptoms, cardiac function, and vascular endothelial function in patients with congestive heart failure. Vascular endothelial function is impaired in subjects with lifestyle-related diseases, such as hypertension, hyperlipidemia, diabetes mellitus, obesity, and smoking. Sauna therapy also improved endothelial dysfunction in these subjects, suggesting a preventive role for atherosclerosis. In animal experiments, sauna therapy increases mRNA and protein levels of endothelial nitric oxide synthase (eNOS) in aortas. In normal-weight patients with appetite loss, repeated sauna therapy increased plasma ghrelin concentrations and daily caloric intake and improved feeding behavior. In obese patients, the body weight and body fat significantly decreased after 2 weeks of sauna therapy without increase of plasma ghrelin concentrations. On the basis of these data, sauna therapy may be a promising therapy for patients with lifestyle-related diseases.

Kanker

Hier vonden we één onderzoek op muizen dat aanwijzingen geeft dat volledige opwarming van het lichaam door infrarood-straling, een gunstige invloed heeft op de behandeling van borstkanker.

  1. Inhibition by whole-body hyperthermia with far-infrared rays of the growth of spontaneous mammary tumours in mice [2000]
    WBH with FIR, however, strongly inhibited the tumour growth without deleterious side-effects, while chronic FIR irradiation itself again had little effect in this process. This WBH regimen may serve as a useful animal model for long-term studies of a noninvasive treatment of breast cancer.
